Terms Of Service
These terms are considered to be accepted when you send us a case.
Prices
Subject to change. Notice will be given in advance where possible.
Case Turnover
We aim to complete most cases in 7 working days from the day the job arrives in the lab. The prices in our most recent publication are based on this service. Implant cases can take longer especially when we have to wait for components to arrive.
Express service is available subject to booking and with a 30% surcharge.
Prescription
For us to adhere with the MHRA guidelines, it is important that your prescription is correctly completed. We have provided tick boxes to speed up the process and also space to provide details.
If no quality level it ticked, it will be assumed that you have prescribed Superior level and charged appropriately.
Dispatch
We cover the whole of the UK. In order to do this and still have an acceptable turnover time without losing cases, we work with a nationwide courier service. Please give us a call to book a pick-up. We will arrange this and then have the job returned to you by courier as well. In this way, we can track all of our packages throughout the UK.
We will still provide a daily local pick-up service to all practices within 15 miles of our location.
Shade Changes
May be chargeable at cost price plus labour depending on level of communication in the given case.
Payment Terms
All accounts are payable 14 days net after end of the month the work is completed.
All new accounts are set up with a variable Direct Debit authority. This must be in place before your first case in invoiced. We partner with GoCardless Ltd to collect Direct Debits and you are at all times protected by the DD Guarantee.
We also accept BACS (bank transfer) payments and all major credit and debit cards.
Interest may be charged on accounts outstanding after this time. In the event that a debt needs to be collected, all legal fees will be added to the outstanding account.
Staged Invoices
Complex cases that involve multiple visits will be billed in stages. The overall cost will not increase. Cases that make no progress for three weeks will be charged up to the stage completed.
Impressions
4D Ceramix reserve the right to return or query impressions or bites we deem doubtful or unsatisfactory. Cases that a dentist has requested that we continue after the laboratory has requested a new impression to be taken will be the responsibility of the dentist.
Quality Control
Cases deemed unsatisfactory by the dentist or patient must be returned to the laboratory for adjustment or remake within 28 days or full charge may be made. The original models and impressions must accompany the case if a FOC remake is requested.
